Arthrodesis is the treatment of choice for either primary instability resulting from inflammatory process or secondary instability caused by late manifestations of pseudarthrosis or sterile spondylodiskitis. Fusion to the occiput is indicated when adequate fixation at C1 is not possible or when there is instability at the occiput-C1 junction.¹
