Surgical decompression may be necessary in those patients unresponsive to conservative treatment (which may include epidural steroid injections). Goals of surgery include central decompression, foraminotomy, and partial facetectomy for lateral recess stenosis. Fusion is recommended for those patients with spondylolisthesis.¹

Back Pain Control (Traction)

Traction is favored by some practitioners in an attempt to obtain symptomatic relief in the treatment of patients with disk disorders. There are proponents of gravity inversion, gravity lumbar reduction, auto traction, and pelvic traction. However, controlled studies do not support the use of traction for long-term control of low back pain.

Depending on the size of the patient, the type of equipment available, and the type of disk pathology, properly applied traction modalities may be useful to provide short-term symptomatic relief in a subset of acute patients.¹
